Customer service representatives are available to help pharmacists identify the beneficiary's plan by providing some basic information.Upon calling this number, pharmacists must identify themselves with their NCPDP Provider ID number.Pharmacists should also have most of the following information available to assist the call service representative with the search: HIC #, Date of Birth, Beneficiary Name, Zip Code, Part A or B Effective Date and Gender. Upon completion of the search, the CMS call service representative will identify the drug plan name and, if requested, the effective date of Medicare coverage.
